large, prominent eyelids - high lid crease

"My eyelids are too large and prominent and they look similiar to someone who would have ptosis excpet for the fact that my pupil isnt covered at all by the eyelid. What can be done to adress this issue? I have heard of fat grafting into the hollow area in the eye, is this the only option?"

A high lid crease, which is what you are descibing can be found in many conditions. The most common is droopiness of the upper lids (ptosis). When one has ptosis the body reflexively tries to open the eyes by raising the eyebrows. This pulls the eyelids upward and increases the distance between the lashes and the crease. People who have upper lid ptosis and reflexively raise the brows will have wrinkles on the forehead. Another common cause is a lack of fat in the region between the eyeball and the bone (superior sulcus). The lack of fat can be secondary to surgical removal or can just occur as we age. The high lid crease is also found in many ethnic groups. The managemnet of the high lid crease obviously depends on the cause. Ptosis repair invariably lowers the crease if the cause is ptosis. Fat pearls or a strip of fat can be placed into the superior sulcus area.Fat can also be injected into the subbrow fat pad which will allow the skin to hang lower and camouflage the high lid crease.
